I ran through the module using a clean install of DNN 4.05.01. The install went fine without any issues.
Once the module was listed in the modules drop down, I added a page and inserted the newly installed module into the page. When the module is first loaded it provides you with a nice description on how to get started using the module. Initially you can use a question wizard to generate your form. One nice feature here is the ability to select from a common set of questions like user information (first, and last name, etc.), so I added a couple simple questions like name information, and some general location questions. Just click the run wizard link and your form is there, simple as pie.
The best part is when you click on a question to edit it. I couldn't believe the amount of options here. I can map questions to DNN fields, add validation, add JavaScript behind a form element, change every attribute I could think of, change CSS styles on the form elements, run events based on values within the form, the options are amazing. Basically everything you could think of doing in the code behind you could do here, except without writing any code.
Even DNN roles come into play with this module. I can hide a question based on a DNN role, providing my users with a unique experience. It's as if every scenario you could think of has been thought of in the development of this module.
Of course a form isn't any good unless you can view the data collected, and this module provides you with the ability to view the results within a grid, or export the results to Excel format.
Question events are new feature here that provide some basic flow to the questions. For example, you can display a question based on a previous selection. This would make the module great for surveys that have follow up questions.
For the form results, you can do more than simply send it to an Excel spreadsheet, you can run some SQL once the form is submitted, or redirect the use to a URL of your choice like a "Thank you for taking our survey..." page.
